Based on the basic story template from Pixar's 22 Rules of Storytelling, I created three small booklets to help a child start writing and illustrating their own stories. The story template goes like this:

 

"Once upon a time there was ___. Every day, ___. One day ___. Because of that, ___. Because of that, ___. Until finally ___."

 

Choose one of the three booklets based on your child's skill level (the booklets range from approx. grade 2 to grade 6 reading level), then print out the booklet. There are two booklets per printing, so cut the papers in half to seperate them. Then guide your child through the fill-in-the blank story. After writing the story, pencil in all the illustrations, then colour them! You will enjoy seeing the creativity that comes out of this exercise.
